Case Management and Rehabilitation

Patient Services: Case Management and Rehab

The everyday complications and challenges brought on by a mental illness should not be underestimated. Andrews Center can help. Case Managers and Rehabilitation Trainers can help people living with a mental illness better manage daily life by linking them with community and financial resources, and by providing important training.

It is easy to become overly concerned about everything when a mental illness is present. It’s just as easy to not care about any thing at all. These are symptoms of the illness, not personality traits. Andrews Center can help make life less frantic, confused and confining. Through counseling and training, one will learn how illness affects lifestyle, how to take medication properly, and how medication helps.

By learning about mental illness and how to identify and cope with symptoms, life can become more enjoyable and rewarding and fulfilling dreams becomes possible.

Rehab Services

Rehab Services provide training and support for patients and their families, helping to educate them about the patient’s diagnosis, medications, monitoring and management of symptoms and side effects. Rehab Services offer psychosocial services, monitoring, assessment of service needs, service planning, and training for daily and independent living skills. Supported housing and co-occurring psychiatric/substance abuse disorder services are also provided if needed. Patients must meet state guidelines to receive these services.

Mobile Crisis Outreach Team (MCOT)

Crisis Hotline: 1.877.934.2131

The Mobile Crisis Outreach Team performs in-person assessments and provides referral services for individuals experiencing mental health crises in a variety of settings in the community, including hospitals, schools, doctor’s offices, law enforcement offices, nursing homes, and more. Once dispatched, the team member will make initial contact and conduct an assessment while also providing crisis intervention and symptom management. Further evaluation and referral and admission services are also a part of this program.

The team works closely with psychiatrists, nurses, and screening and counseling staff and makes appropriate referrals to Andrews Center and to other community agencies.

Face-to-face crisis assessments are available 24 hours a day by calling the 24 Hour Hotline at 1.877.934.2131.

Hope House

Hope House is a Crisis Respite Program working in partnership with Andrews Center. The program was established as a “step down” from a hospital stay, or as a means to begin or continue stabilization for persons who have experienced a mental health crisis in recent days.

Hope House is a 12-bed facility and admissions are voluntary. It provides psychiatric, medication management, and psychosocial services to individuals seeking and desiring help. The length of stay is determined by the amount of time it takes a client’s crisis to end, but the recommended stay is up to 14 days.

Hope House is operated by The Wood Group.
